Please provide the docket number from your separation agreement which is usually found on the front page of the agreement. Please also include the county where your divorce was granted.

    Sometimes the Plan Administrator will charge a review fee to review the QDRO on their end. This is separate from my fee and will be taken out of the account balance in accordance to your separation agreement.
